RiseWell Community Services is seeking a Full-Time Housing Coordinator in Hauppauge, NY to support the Adult SPOA unit within the Suffolk County Department of Health. The role focuses on coordinating and facilitating access to housing programs for adults with behavioral health conditions, reviewing applications, linking individuals to resources, and ensuring policy adherence.

Strong organizational and communication skills are essential.

Qualifications

• Bachelor’s degree, or equivalent experience.
• Experience in project management or related field preferred.
• Strong organizational and communication abilities.

Responsibilities

• Coordinate and facilitate access to housing programs for individuals with behavioral health conditions.
• Review and process applications for respite housing, SPA housing, and Crisis Residence.
• Link referred individuals to appropriate resources and coordinate with providers.
• Ensure policies and procedures are followed and work with referral sources to optimize system performance.
