RiseWell Community Services is seeking a Full-Time Sr. Housing Coordinator to join the Suffolk County, Department of Health, Division of Mental Hygiene, Adult SPOA Unit in Hauppauge, NY.

The role focuses on coordinating and facilitating access to housing program opportunities for individuals with behavioral health conditions, serving as the primary County contact and ensuring policies and procedures are followed. Strong Excel skills and organization are essential.

Qualifications

• Bachelor’s degree, or like experience required; project management or similar experience preferred.

Responsibilities

• Coordinate and facilitate access to housing programs for individuals with behavioral health conditions in Suffolk County.
• Review and process applications for adults seeking respite, Crisis Residence and/or SPA housing.
• Serve as the primary County contact for oversight of housing programs and liaise with providers and referral sources.
• Maintain reports and statistics using Excel to identify trends in housing systems.
