Job summary

Step Up’s Partners for Home Rapid Housing Initiative Permanent Supportive Housing Program in Georgia seeks a qualified Case Manager to provide mental health services and care coordination to adults 18+ in community settings, linking clients to housing, benefits and wellness resources. The role emphasizes Housing First principles and trauma-informed care within a supportive team.

You will conduct assessments, develop care plans, attend on-site and community meetings, and advocate for clients

Qualifications

• Master's degree in human services and license eligible.
• Experience with mentally ill adults and homelessness.
• Strong case management and documentation skills.
• Ability to be on-call as assigned.
• Must have transportation and Georgia driver's license.

Responsibilities

• Assess homeless adults for mental illness, co-occurring substance use and other health care needs in the community until linkage and services are accepted.
• Attend staff and community meetings as requested by Program Manager.
• Engage in supervision with management to discuss caseload and documentation.
• Complete initial and ongoing assessments for diagnosis and functioning.
• Provide clinical therapeutic and case management services to assist functioning in the community.
• Attend medical or mental health appointments with clients to advocate and ensure coordinated care.
• Make home visits to support independent living and housing retention.
• Monitor for compliance, stability, and decompensation signs.
• Develop Coordinated Care Plans with set timeframes and goals.
• Outreach clients on caseload to ensure engagement in services.
• Provide individual, group, and crisis interventions as needed.
