Job summary

Step Up in Decatur, Georgia, seeks a qualified professional to lead and administer the Fulton Permanent Supportive Housing Program, delivering clinical therapy and case management to adults with psychiatric disabilities in the community. You will assess needs, coordinate services, and advocate for housing retention, completing required documentation in an on-site setting.

The role requires on-call rotation, transportation, and a Georgia driver's license.

Qualifications

• Master's degree in human services and license eligible.
• Experience with mentally ill adults and chronic homelessness.
• Case management experience essential.
• On-call rotating schedule; Georgia driver's license and insurance.

Responsibilities

• Provide clinical therapy and case management in the community.
• Attend medical/mental health appointments with clients.
• Assist with housing retention and skills development.
• Complete paperwork in EHR and maintain charts.
• Engage in outreach to clients on caseload.
• Ensure safety and crisis de-escalation when needed.
