Job summary

Step Up in Atlanta seeks a dedicated professional to provide clinical therapy and intensive case management for adults with psychiatric disabilities in the community. The role emphasizes linkage to housing resources, benefits, and wellness services through a Housing First approach.

You will conduct assessments, participate in supervision, and advocate for coordinated care, including 24/7 on-call support and detailed documentation to ensure continuous, person-centered care.

Qualifications

• Master's degree in human services and license eligibility.
• Experience with mentally ill adults and chronic homelessness.
• Case management experience and ability to document progress.

Responsibilities

• Assess homeless adults for mental illness, co-occurring issues, and housing needs.
• Attend staff and community meetings as requested by Program Manager.
• Provide therapy and case management to improve community functioning.
• Make home visits to support independent living and housing retention.
• Develop Coordinated Care Plans with timelines from enrollment.
• Monitor for crisis and decompensation; outreach on caseload.
• Document via intake forms, treatment plans, and service notes.
