Step Up, Fulton Permanent Supportive Housing Program in Georgia, seeks a licensed-eligible human services professional to provide therapeutic and case management services to adults with psychiatric disabilities in community settings.

You will conduct assessments, coordinate care, and advocate during medical and mental health appointments, supporting housing retention and community integration.

Qualificações

• Master's degree in human services and licensure eligibility.
• Experience with mental illness and homelessness in community settings.
• Experience in case management and charting for documentation.

Responsabilidades

• Assess homeless adults for mental illness and related needs in the community.
• Attend staff and community meetings as requested by Program Manager.
• Engage in scheduled supervision to discuss caseload and documentation.
• Provide clinical therapy and case management to improve functioning.
• Attend medical or mental health appointments to advocate for integrated care.
• Perform all other duties as assigned.
