Job summary

Step Up in Decatur, GA is seeking a Permanent Supportive Housing Case Manager to provide intensive housing support for adults with chronic homelessness. The role focuses on housing stability, service coordination, and community resource linkages in Fulton County’s Housing First program.

Responsibilities include intake, assessment, treatment referrals, and coordinating with providers to maintain permanent housing.

Qualifications

• Bachelor's Degree required or equivalent experience in housing services.
• Certified Peer Specialist preferred, not required; HS diploma acceptable.
• Proficient use of Office 365 products.

Responsibilities

• Provide engagement and housing stability support to a caseload up to 25.
• Conduct intake and assessment, develop service plans, referrals and leasing support.
• Meet members via home visits or telehealth and monitor progress toward housing stability.
• Coordinate with community providers to ensure ongoing clinical supports.
• Document services using BIRP notes and HMIS as required.
