Job summary

Step Up is seeking a Housing Case Manager II to provide Medicaid Billable outreach, engagement, intake, housing search/placement and ongoing support to adults experiencing chronic homelessness in the Atlanta area.

The role operates within a Housing First team, emphasizing Trauma-Informed Care, Harm Reduction, and Recovery Oriented services, with in-person and telehealth visits to support housing stability.

Qualifications

• Bachelor’s degree from an accredited college or university.
• Associate’s degree and minimum 2 years of human services experience as listed in the required experience section below.

Responsibilities

• Provide Medicaid Billable intensive case management Housing First services to a caseload of 25-40 chronically homeless persons to obtain and maintain Permanent Supportive Housing.
• Meet with assigned members via telehealth and in‑person home visits to review progress toward goals and provide Medicaid billable case management and skill-building interventions.
• Coordinate care with community providers to ensure housing stability.
• Provide transportation and support to teach independent living skills.
