Job summary

River Edge Behavioral Health in Atlanta, GA is seeking a Full-time Housing Outreach Care Coordinator to support Fulton Permanent Supportive Housing clients. You will work with the Case Manager to assist with housing applications, referrals, and coordination of services in Fulton County.

The role requires a Master's in a related field, licensure eligibility, and 2+ years of post-master's experience in mental health/substance abuse treatment. Background check and drug testing are required.

Qualifications

• Master's degree in social service field (e.g., Social Work, Sociology, Psychology) or related field such as counseling.
• License eligible in LAPC, LMSW, LAMFT, LMFT, LPC, LCSW.
• Fully Licensed or Associate Licensed in LAPC, LMSW, LAMFT, LMFT, LPC, LCSW.
• At least two years of postmaster's experience providing mental health and substance abuse treatment.
• Demonstrate knowledge and expertise with the chronically mentally ill population.
• Experience in Permanent Supportive Housing is a plus.
• All applicants must pass a satisfactory background clearance and pre-employment drug test.
• Provide 2 professional reference statements (former employers only).
• Demonstrate expertise in mental health and substance abuse services and community resources.
• Exemplify excellent communications skills, both verbal and written.

Responsibilities

• Work closely with the Case Manager to support individuals in permanent supportive housing.
• Maintain a caseload of up to 20 individuals and conduct in-home visits.
• Conduct confidential behavioral health assessments to determine level of care.
• Develop solution-focused treatment plans with client participation.
• Participate in individual and group counseling and court attendance if applicable.
• Identify and intervene in potential crisis situations.
• Attend staff meetings, case conferences, and training programs.
• Attend clinical supervision to enhance skills.
• Provide clinical consultation to referral resources, family members and others.
• Maintain current clinical records, treatment plan reviews and outcomes.
• Maintain knowledge of treatment agencies and resources available to clients.
• Ensure compliance with River Edge policies and licensing requirements.

Education

Master's degree in social service field

License eligible: LAPC, LMSW, LAMFT, LMFT, LPC, LCSW

Fully Licensed or Associate Licensed: LAPC, LMSW, LAMFT, LMFT, LPC, LCSW

2+ years post-master's experience in mental health & substance abuse

Knowledge of chronically mentally ill population

Experience in Permanent Supportive Housing (a plus)

Background clearance & pre-employment drug test

2 professional references (former employers only)

Knowledge of mental health & substance abuse services and community resources
