Claratel Behavioral Health in Decatur, GA is seeking a Housing Support Case Manager to help adults in permanent subsidized housing achieve stability and wellness through comprehensive case management and psychosocial rehabilitation. You will carry a caseload of 25-30 clients, conduct regular visits, coordinate services, and support clients with housing search, safety checks, and lease processes.

The role requires at least a Bachelor's degree in a related field, one year of behavioral health

• Bachelor’s degree in related field or equivalent education and experience.
• One year of experience in behavioral health case management.
• Proficient in EMR, telehealth, and mobile devices.
• Strong care coordination and teamwork skills.
• Reliable transportation to work.
