Urban Pathways is seeking a Case Manager to provide and document clinical and case management services for assigned clients. The role emphasizes active listening, high-quality customer service, and cross-cultural awareness to promote an inclusive anti-racist organizational culture.

The position involves field work, home visits, and outreach, with responsibilities spanning intake, assessments, service planning, and collaboration with housing, health, and employment resources.

Competenze

• Bachelor’s degree required. Bachelor’s degree in Social Work or other related field preferred.
• Minimum one year of case management, individual or group counseling experience preferred.
• Knowledge of NYC housing subsidy programs.
• Experience with people experiencing homeless, mental illness and/or substance abuse population preferred.

Mansioni

• Provides comprehensive case management services for assigned clients including referrals to meet objectives.
• Develops service plans and/or service referrals to achieve client self-sufficiency.
• Conducts home visits, outreach, and field work as required.
• Maintains timely documentation, progress notes, intake forms, and client records.
• Assists clients with crisis intervention and advocacy as needed.
