Job summary

MetroPlusHealth is seeking a Housing Specialist to support members experiencing homelessness and to optimize their health care with housing goals. You will conduct assessments, coordinate with DHS/HRA, and assist with housing applications through telephonic and field outreach as part of a multidisciplinary care team.

The role involves field visits within the MetroPlusHealth service area and requires strong collaboration, advocacy, and problem-solving to ensure access to housing and health

Qualifications

• Associate’s degree and four (4) years of experience processing housing applications, including NYCHA, HRA, and NY/NY programs.
• Bachelor’s degree and two (2) years of relevant experience.
• High school diploma or GED with six (6) years of relevant experience.

Responsibilities

• Engage in telephonic and field-based outreach to vulnerable members.
• Assist members with housing applications and coordination with landlords and housing providers.
• Coordinate with DHS, HRA, and ACT to facilitate housing goals and advocate for members.
