Job summary

Qualifications

• Utah Case Management Certification or Utah SSW license in good standing.
• Valid driver's license with ability to travel to client visits and facilities.
• Utah resident living in Logan, UT.
• Ability to meet weekly in-person client contact requirements and travel within the area.
• Strong written and verbal communication; organized documentation.
• Work independently and meet deadlines; professional conduct.

Responsibilities

• Maintain weekly face-to-face contact with clients and assess housing needs and barriers.
• Coordinate housing services and connect clients with resources and supports.
• Monitor progress toward stable housing and follow up on needs.
• Build relationships with clients, families, providers, and community partners.
• Travel within the county for client visits and coordinate with facilities and teams.
• Document services and maintain accurate records for Medicaid billing and compliance.
