Job summary

Health Care for the Homeless is seeking an RN Case Manager to provide medically-related case management services within the Housing Services program. You will coordinate medical care, link clients to health resources, and support medication adherence while conducting nursing assessments and treatments.

This onsite role requires trauma-informed care, client-centered planning with SMART goals, HIPAA-compliant documentation, and collaboration with a multidisciplinary team to improve housing

Qualifications

• Bachelor’s Degree from an approved School of Nursing.
• Licensed in Maryland as a Registered Nurse.
• Valid Maryland driver's license and access to a personal vehicle.

Responsibilities

• Provide nurse case management services including medical care coordination and linkage to health resources.
• Deliver health education and counseling to clients using a client-centered, trauma-informed approach.
• Perform nursing assessments, treatments, medication administration and adherence support.
• Conduct home and community visits, and document in the electronic health record with required timeframes.
• Collaborate with multidisciplinary team to improve housing placement outcomes.
