Responsibilities

• Participates in outreach services to prospective clients
• Coordinates outreach efforts with outside agencies
• Provides peer counseling and support to validate clients' experiences and encourage participation in recovery
• Mentors clients to promote hope and empowerment
• Participates in ongoing psychosocial assessment of goals and needs
• Develops housing rehabilitation plans and helps clients find and maintain housing
• Completes program data and reporting for PATH activities
• Assists clients in obtaining housing and related services
• Ensures confidentiality, safety, and client-centered care
