Job summary

City Net is seeking a Case Manager to provide housing-focused case management and supportive services to individuals and families experiencing homelessness. You will meet clients where they are, help them develop long-term housing plans, and connect them with essential resources including medical and mental health care.

Responsibilities include housing placement, client needs assessments, advocacy, data collection, and ensuring compliance with program contracts.

Qualifications

• Excellent verbal and written communication skills in a professional context.
• Ability to build and maintain productive relationships with stakeholders.
• Strong data collection and reporting discipline for HMIS/CES data.

Responsibilities

• Meet clients through multiple interactions to prioritize housing and services.
• Help clients access housing programs and complete housing applications.
• Coordinate referrals to medical, mental health, and supportive services.
