Job summary

City Net in Downey, CA is seeking a Case Manager to deliver housing-focused case management and supportive services to individuals and families experiencing homelessness, primarily through street outreach. You will meet clients where they are and help them develop long-term housing plans.

This role collaborates with nonprofits, government entities, and health providers to coordinate care, connect clients with resources, and document data for HMIS and CES while meeting productivity goals.

Qualifications

• Eligible to drive a City Net vehicle (21+ with valid license).
• Empathetic, self-motivated, flexible, and able to work independently or as part of a team.
• Experience building relationships with stakeholders.
• Strong data collection and reporting skills.
• Excellent professional boundaries and communication skills.
• Ability to work in variable scheduling including evenings/weekends.

Responsibilities

• Conduct client needs assessments and build trust through multiple contacts.
• Assist clients with housing program applications and long-term housing plans.
• Coordinate linkage to medical, mental health, and supportive services.
• Advocate with partner agencies and accompany clients as needed.
• Collect and document data for HMIS and CES systems.
• Ensure productivity goals and compliance with contracts and policies.
