Job summary

City Net is seeking a Case Manager to provide housing-focused case management and supportive services to individuals experiencing homelessness, primarily through street outreach. You will meet clients where they are to help develop long-term housing plans and connect them with resources for medical care, counseling, and permanent housing.

The role follows a four/10 schedule, working Wednesday through Saturday, 7am–5:30pm, and requires strong data collection, advocacy, and collaboration with

• Experience cultivating and maintaining professional relationships with diverse stakeholders.
• Understanding of data collection and timely reporting.

Responsibilities

• Assess client housing and supportive service needs.
• Assist with housing program applications and long-term housing plans.
• Refer clients to medical, mental health, and other services.
• Advocate for clients with partner agencies to access resources.
• Collect and document data for HMIS and CES.
• Meet productivity and outcome goals while complying with contracts.
