Job summary

City Net is seeking Case Managers to provide housing-focused oversight for individuals and families experiencing homelessness. You will conduct street outreach, assess housing needs, and connect clients to medical, mental health, and housing support services in partnership with nonprofits, government entities, and healthcare providers.

Typical schedule is Monday–Friday, 7:30am–4:00pm, with opportunities to impact housing outcomes.

Qualifications

• Must be eligible to drive a City Net vehicle and meet insurance requirements.
• Strong empathy and desire to help vulnerable neighbors.
• Professional boundaries and effective communication are essential.
• Ability to build productive relationships with diverse stakeholders.

Responsibilities

• Client Needs Assessment and outreach to prioritize housing and services.
• Housing Placement: develop and execute long-term housing plans.
• Linkage to Supportive Services: referrals to medical, mental health, etc.
• Advocacy: coordinate with partner agencies to access resources.
• Data Collection: accurately document client data in HMIS/CES.
• Compliance: meet productivity and outcome goals and follow procedures.
