Job summary

Homes with Hope, Inc. CT in Westport, CT is seeking a full-time Case Manager to support residents of the Gillespie Center and Hoskins' Place, linking housing, services and resources under a Housing First model.

The role includes coordinating care, maintaining shelter operations, and advocating for clients to achieve housing stability. Responsibilities include assessments, individualized action plans, referrals, information releases, and collaboration with medical and social services.

Qualifications

• BSW degree or higher from an accredited college or university in the social sciences; or related degree.
• Minimum of two years’ experience in case management services preferred.
• Motivated with sense of teamwork.
• Good communication and people skills.
• Demonstrated skills in working with clients from various backgrounds.
• Embrace the agency's mission to provide basic human needs with compassion and sound ethics.
• Demonstrated proficiency in MS Word, Excel, and Internet usage.

Responsibilities

• Assess client's situation/needs.
• Determine individual housing goals.
• Develop an individual case action plan to help resident meet needs and housing goals.
• Provide referrals to resources including benefits, treatment, detox, medical concerns, and housing.
• Secure release of information and consent forms for outside services.
• Coordinate activities from the action plan with clients and other staff.
• Monitor adherence to medication and coordinate with physicians as needed.
• Plan transportation for appointments and housing-related transports.
• Re-evaluate housing plan as situations change.
• Advocate for clients to obtain services or benefits.
• Intervene during crises affecting housing plan.
• Document case notes and update CT HMIS and external systems.
