Job summary

Homes with Hope, Inc. in Westport, CT seeks a full-time Case Manager to support residents of the Gillespie Center and Hoskins Place shelter. You will assess needs, develop action plans, and connect clients with housing resources and benefits, while ensuring program operations and confidentiality.

The role emphasizes teamwork, strong communication, and adherence to the Housing First approach to rapidly move clients toward housing. A CT driver’s license is required.

Qualifications

• BSW degree or higher from an accredited college or university in the social sciences or related field.
• Minimum of two years' experience in case management services preferred.
• Bi-lingual/Bi-cultural a plus.
• Valid CT driver’s license and insured automobile.
• Demonstrated proficiency in MS Word, Excel, and Internet usage.

Responsibilities

• Work with residents of the emergency shelter to address housing barriers using a Housing First service model.
• Coordinate case actions to help residents meet housing goals.
• Provide referrals to community resources including benefits counseling, substance abuse treatment, and housing services.
• Maintain CT HMIS and other client information systems.
• Ensure program adherence to confidentiality and agency policies.
