Job summary

Mid-Maine Homeless Shelter & Services in Waterville, Maine, is seeking a Full-Time Targeted Case Manager. You will work within a low-barrier shelter to help guests transition from homelessness to permanent housing, using Housing First, trauma-informed care, and harm reduction principles.

This role includes delivering MaineCare Targeted Case Management services (Section 13/17) for eligible participants and collaborating with a multidisciplinary team to reduce barriers and promote recovery.

Qualifications

• Bachelor's degree in Social Work, Human Services, Psychology, Sociology, Behavioral Health, or related field or MHRT-C certification.
• Valid driver's license and reliable transportation.
• Strong communication, organizational, and documentation skills.
• Ability to work independently while collaborating effectively within a multidisciplinary team.

Responsibilities

• Develop individualized service and housing plans reflecting guests' strengths and needs.
• Provide case management using Housing First, trauma-informed, and harm reduction principles.
• Deliver Targeted Case Management services (Section 13/17) for eligible participants.
• Help guests obtain permanent housing, income, benefits, healthcare, behavioral health services, transportation, and community supports.
• Advocate for guests while coordinating with landlords, housing providers, healthcare organizations, and partners.
• Support guests as they transition into housing and provide follow-up for long-term retention.
• Maintain accurate documentation in HMIS and health records.
