Job summary

Community Care ME in Bangor is seeking a Permanency Specialist to support individuals experiencing homelessness as they transition into and maintain permanent housing. Using Housing First and trauma-informed approaches, you will build life skills, connect to resources, and promote housing stability in participants’ homes and communities.

The role emphasizes relationship-building, advocacy, and collaboration with clinicians and case managers to ensure coordinated care.

Qualifications

• Lived experience with homelessness strongly preferred.
• At least 1 year of direct service experience with people experiencing homelessness.
• Experience in direct support, case management, housing services, or human services preferred.
• Strong interpersonal and communication skills.
• Ability to work independently and in a team.
• Commitment to Housing First principles, trauma-informed care, and culturally responsive delivery.
• Maintain professional boundaries and accurate documentation.
• Valid driver’s license and reliable transportation.

Responsibilities

• Provide in-person support to participants in homes and community settings.
• Build trusting relationships and encourage engagement with services.
• Assist with daily living tasks, housing stability, employment, education, and personal goals.
• Support housing transitions, move-ins, furnishing, and settling into new housing.
• Conduct regular check-ins to assess needs and provide ongoing support.
• Facilitate skill-building activities and supportive groups.
• Accompany participants to appointments when requested.
• Assist with transportation and access to resources.
• Maintain positive landlord relationships and address housing concerns.
• Document contacts and progress timely.
• Collaborate with case managers for coordinated care.
• Use Motivational Interviewing and trauma-informed practices.
