Responsibilities

• Provide intensive case management to tenants under Housing First-based programs.
• Assist tenants with obtaining benefits, health care, education, employment, and legal aid.
• Develop individual service plans and maintain documentation for all tenants.
• Collaborate with housing staff on program-wide efforts to stabilize tenants.
