Job summary

Pine Street Inn is seeking a Consolidated Tenant Case Manager in Boston to provide intensive case management for chronically homeless individuals with complex needs. You will work within a housing-first framework, applying harm-reduction and motivational interviewing to promote housing stability and client growth.

The role requires completing necessary program documentation, assisting tenants with benefits and services, and coordinating with housing staff to support tenants’ daily living

Qualifications

• Must have clear commitment to the population served and be able to work as part of a team.

Responsibilities

• Provide comprehensive, intensive case management for chronically homeless individuals with histories of substance abuse and mental illness in housing.
• Use harm-reduction and trauma-informed approaches; apply Stages of Change Theory and Motivational Interviewing techniques.
• Maintain a caseload and assist tenants with program documentation, benefits, healthcare, education, employment, and legal aid.
• Support development of tenants’ daily living skills, create individual service plans, maintain documentation; monitor maintenance needs and assist with program fee collection.
