Job summary

Boston Rescue Mission in Boston, MA seeks a dedicated case manager to support individuals facing homelessness. You will screen clients, refer them to housing, healthcare, benefits, day programs, and job training, and assist with life skill development.

Responsibilities include documenting encounters, coordinating services, and monitoring progress. A Bachelor's degree or two years in social services is required, with full- or part-time hours available.

Qualifications

• Bachelor's Degree or at least two years' experience in housing, health or social services.
• Strong interpersonal and communication skills to work with diverse communities.
• Ability to document, track and report client progress using standard formats.

Responsibilities

• Completes detailed screening and intake to make referrals to appropriate case management services.
• Facilitates access to permanent housing, healthcare, benefits, employment, and training.
• Assists guests in improving life and coping skills and reconnecting with family.
• Coordinates and monitors in-house referrals and supports goal progress.
• Documents all client encounters and maintains comprehensive files.
