Job summary

Catholic Charities of Oregon is seeking a skilled case manager to support families at Good Shepherd Village. The role focuses on intake, needs assessments, resource connections, and ongoing case planning to promote housing stability and income growth.

Responsibilities include facilitating Rent Well and other enrichment programs, coordinating with Resident Services, and maintaining timely documentation. Travel between Portland sites is required.

Qualifications

• Bachelor’s degree in human services, social work, early childhood education or equivalent lived/worked experience.
• Master’s degree in Social Work or related field preferred, with 3 years’ experience with diverse populations and families in crisis.
• 1 year case management required; 3+ years preferred in homelessness/PSH settings.
• Experience with vulnerable families facing homelessness, trauma, mental illness, addictions, and chronic medical needs.

Responsibilities

• Provide evidence-based case management services to 15-20 families at Good Shepherd Village.
• Complete initial intake and assessments to connect residents with services.
• Assist residents in applying for long-term income sources and benefits.
• Use Housing First and harm reduction approaches to support housing stability.
• Attend quarterly trainings to improve service delivery and cultural responsiveness.
• Coordinate with on-site Resident Services and property management.
• Document services within 24 hours of meetings or case developments.
• Meet program goals including housing stability and income growth.
• Lead Rent Well classes and other adult enrichment activities.
• Travel between sites in Portland and maintain a flexible schedule.
