Job summary

Catholic Charities of Oregon is seeking an Adult Permanent Supportive Housing Case Manager in SE Portland. The role focuses on delivering comprehensive case management for families in PSH, implementing individualized plans, and coordinating services with on-site staff and community partners.

You will lead classes and group activities while supporting residents facing homelessness, trauma, and health challenges.

Qualifications

• Bachelor’s degree in human services, social work or equivalent lived/worked experience required.
• Master’s degree preferred with 3+ years of diverse populations experience in crisis.
• 1 year case management required; 3+ years in social services or PSH preferred.
• Experience with homelessness, trauma, mental illness, addictions, and chronic health needs.

Responsibilities

• Provide evidence-based case management for 15–20 families in PSH.
• Complete intake and assessments to connect residents with services.
• Assist residents in obtaining income through programs like OHP, TANF and SNAP.
• Apply Housing First and harm reduction to support housing retention.
• Lead on-site classes, groups, and events for residents.
• Document services and referrals within 24 hours of contact.
