Job summary

Catholic Charities is hiring an Adult Permanent Supportive Housing Case Manager in SE Portland. You will lead case management for families in PSH, provide crisis intervention, and support housing stability through individualized plans.

The role involves coordinating with property management, resident services staff, and community partners, and delivering group classes and events for residents. A strong background in homelessness and trauma is preferred.

Qualifications

• Bachelor’s degree in human services, social work, or equivalent lived/worked experience required.
• Master’s degree in Social Work or related field preferred, with 3+ years experience with diverse populations and families in crisis.
• 1 year case management required; 3+ years preferred in social services or PSH settings.
• Experience with homelessness, trauma, addiction, and chronic health needs; crisis response ability.
• Proficiency with standard office tools (Word, Excel, Outlook, PowerPoint).

Responsibilities

• Provide evidence-based case management to 15–20 families in PSH.
• Complete intake and assessments to connect families with services.
• Assist residents in applying for income sources including OHP, TANF, SNAP, Head Start.
• Apply Housing First and harm reduction approaches to support housing retention.
• Attend quarterly trainings to enhance knowledge on resident needs.
• Collaborate with on-site management and partner agencies to optimize outcomes.
• Document services, referrals, and progress in the electronic record within 24 hours.
• Maintain flexible schedule to meet program needs.
• Coordinate on-site enrichment activities and family events.
