Job summary

Qualifications

• Post-secondary education in social services with 1 year of direct crisis services.
• Background checks are required prior to employment.
• Ability to document client information and maintain records in HMIS.
• Excellent written and oral communication skills.

Responsibilities

• Conduct strengths-based housing needs assessments and develop service plans with Participants.
• Maintain accurate case files and notes per standards.
• Help Participants pursue housing and employment goals and financial coaching.
• Provide crisis de-escalation and advocacy to support progress toward goals.
• Coordinate services with CCS staff and external partners to ensure coordinated care.
• Input and track participant data in HMIS and assist with program reporting.
