Job summary

Lakes and Pines CAC, Inc. is seeking a Housing Coach to assist clients facing eviction or homelessness in achieving housing stability. The role involves guiding clients, providing referrals to community resources, and creating personalized case plans focused on building skills and financial stability.

The position requires a High School Diploma or equivalent and a valid driver’s license. Prior experience is not strictly required, but strong communication and organizational abilities are

Qualifications

• Excellent verbal and written communications are required.
• Ability to manage time to balance quantity and quality of work.
• Demonstrated ability to work independently and as part of a team.

Responsibilities

• Build and maintain community relationships to identify services and coordinate benefits.
• Assess customer eligibility for assistance and develop individual goals with the client.
• Document interactions timely and accurately in the agency data system.
• Coach customers through personalized plans and celebrate progress.
• Prepare referrals to internal and external programs and maintain confidential case records.
• Participate in outreach events and share opportunities with the Communications Specialist.
• Ensure compliance with agency policies and program requirements.
