TITLE: Housing Coach

The Housing Coach will assist customers who may be facing eviction or experiencing homelessness to achieve housing stability by providing guidance, supportive services, and referrals to community support systems. The Housing Coach will work with customers to create a personalized case plan which will focus on building skills, strengths and community connections in order to increase financial stability.
High School Diploma or equivalent
Education may be waived with significant job related experience

Excellent oral and/or written communications skills.
Ability to manage time that results in both a satisfactory quantity and quality of work.
Support the Mission of the Agency through daily work and while representing the Agency in public.
Have general knowledge of programs and activities of the Agency.
Demonstrated ability to work independently and as a team member.
Must be able to handle diverse work problems on a daily basis.
Must be able to organize a work schedule, set priorities and work effectively to meet deadlines without supervision.
Must be able to adjust to changes necessitated by last minute assignments and deadlines requests that may prove stressful.

Attend trainings, classes and meetings as necessary to improve job skills and maintain an acceptable level of job performance.
Serve as a mentor for new staff.
Have knowledge and apply Agency policies and procedures, including safety, technology, data practices and other applicable policy, procedure and program regulations.
Understand numbers and mathematical concepts.
Knowledge of public and private sources of debt equity and subsidy.
Knowledge of Community resources.

Build community relationships with other agencies to identify services, coordination of benefits and affect change.
May determine customer eligibility for assistance in some programs.
In conjunction with the customer, develop and monitor individual and household goals.
Conduct regular check-ins on a weekly, bi-weekly or monthly basis depending on customers need and expectations; timely, thoroughly and accurately document all customer interactions.
Respectfully, professionally and effectively coach the customer through the development plan and celebrate the small successes.
Build a network with the customer that includes supports from outside agencies to ensure prosperity.
Prepare referrals to internal programs and outside service providers; engaging professionally and respectfully with all involved.
Annually and as notified of changes, update current listing of community resources to be shared with the agency.
Prepare and maintain confidential case records and files: Timely and accurately document all interactions with customers in the Agency/Program data management system.
Identify and participate in outreach events that brings awareness to the communities of the opportunities the agency offers, and track outreach events using the Agency system. Communicate opportunities to the Communication Specialist.
Proficient computer skills and knowledge of Microsoft Office programs including Work, Excel, Power Point.
Ability to effectively assess situations, prioritize solutions and make appropriate and timely decisions.
Training or experience providing Person Centered Coaching.
Recognize existing and emerging needs of customers, document available community resources to improve service collaboration. Act as an advocate to elevate client challenges to supervisors/coordinators/managers, and help shape responsive program development.
Assess and recognize need for professional development. Research the availability of professional development opportunities to address emerging trends and issues. Communicate to the Director and request applicable training support.
Conduct activities appropriate to the goals of the Agency and department as required.
Actively collaborate with team members, volunteers, program participants and/or community partners in working towards achieving program goals and objectives.
Inform the supervisor and/or Department management of potential problems of programming dealing with procedures and policies.
Entering participant data into data collection systems.
Understand and ensure rights and responsibilities of the agency and the customer.
Staying abreast of updates & changes in programing requirements maintained by other departments in Lakes & Pines.
Actively participate and assist in assigned program, community, committees
