Nelson Connects in Lakeport, CA is seeking an experienced Case Manager to provide housing-related services to families, at-risk adults, and individuals experiencing homelessness through grant programs. You will conduct intake, develop service plans, coordinate with agencies, and maintain case files.

The role requires strong organizational and communication skills, a valid driver's license, and the ability to complete a 40-hour Domestic Violence training within six months.

Qualifications

• Associates or 2+ years related case management experience with possibly a high school diploma.
• Experience delivering direct services in social services environment.
• Strong organizational, communication, documentation, and problem-solving skills.
• Ability to handle confidential information professionally.
• Proficiency with MS Office and electronic data/case management systems.
• Valid driver’s license and satisfactory driving record.
• Ability to complete 40-hour Domestic Violence training within six months of hire.
• Ability to pass background check, fingerprinting, and drug screening (federal cannabis standards observed).

Responsabilités

• Conduct client intake assessments and develop individualized service plans.
• Provide ongoing case management, housing assistance, rental support, and benefits advocacy.
• Help clients locate housing and work with landlords to secure placements.
• Assist with housing applications and supportive services.
• Develop SMART goals for stability and self-sufficiency.
• Coordinate services with internal departments and external agencies.
• Maintain accurate case files, progress notes, and program documentation.
• Track client progress and ensure grant/budget compliance.
• Prepare and submit required grant reports.
• Maintain confidentiality and participate in meetings and training.
