Job summary

Main Salvation Army in Los Angeles, CA seeks a Case Manager to assess housing, health, financial and psychological needs and develop individual service plans aimed at permanent housing and cost-effective care.

This role provides strength-based case management, manages HMIS data, coordinates services with partners, ensures HIPAA compliance, and reports to the Program Manager. Responsibilities include intake, housing plans, and discharge planning, with a focus on community resource collaboration.

Qualifications

• Bachelor's in Social Work preferred or Associate's Degree in related field.
• Possess a valid California Driver's License.
• Minimum of one year experience with low-income and/or homeless individuals/families.
• Must pass a criminal background check.
• If working near children, Protect the Mission policies and procedures certification required.

Responsibilities

• Provide strength-based case management and service coordination to assist clients in obtaining and maintaining permanent housing; including intake, housing plans, case conferencing, and discharge plans with follow ups.
• Maintain and update client information as required by HMIS and ensure accuracy of records and data for reports.
• Compile periodic reports as required by TSA and funding agencies.
• Develop individual service plans for each client and monitor progress via case management sessions; conduct re-evaluations as needed.
• Assist clients with access to financial entitlements (CalWORKs, GR CBEST) and health care; provide advocacy as necessary.
• Monitor and enforce program policies using evidence-based practices such as intensive case management and housing first.
• Gain and maintain comprehensive HMIS knowledge for data entry and reporting.
• Understand local/federal housing program rules (Rapid Re-Housing, Section 8, etc.).
• Drive clients to housing appointments and assist with housing documentation.
