Job summary

Mercy House in Riverside, CA is seeking a Housing Solutions Case Manager to join the Permanent Housing Team. You will conduct needs assessments, develop individualized housing plans, provide support and referrals, and advocate for residents in Mercy House Permanent Supportive Housing programs.

You will screen prospective clients for eligible services and coordinate life skills workshops across multiple facilities.

Qualifications

• Bachelor’s Degree in Social Work (or related field) required
• Experience in chemical health, mental health, or co-occurring disorders preferred
• Spanish fluency is of significant value

Responsibilities

• Maintain a minimum caseload of 25-30 residents and develop housing plans addressing barriers with weekly meetings
• Apply harm reduction methods to keep residents in housing
• Practice Trauma-Informed Care when interacting with clients
• Assist residents with eviction prevention and access to community resources
• Visit resident apartments to ensure maintenance and safety
• Coordinate life skills workshops on job search, parenting, time management, health and wellness
• Coordinate job readiness workshops to support residents in job skills development
• Design and coordinate holiday and community events
• Develop and maintain client records and documentation
