Qualifications

• Bachelor’s degree in social work, psychology, social welfare, or a related field OR two years of experience in a related residential or housing program.
• Minimum of two years’ experience working with under-resourced or low-income populations; experience with homelessness preferred.
• Bilingual (English/Spanish) preferred.

Responsibilities

• Incorporate Cal AIM care coordination and housing navigation strategies into individualized care and housing plans in collaboration with healthcare, behavioral health, and community-based partners.
• Provide housing-focused case management for individuals and in group settings.
• Identify and address barriers to housing, including homelessness, health needs, and income gaps.
• Documentation of care plans, progress notes, and housing goals in HIPPO and HMIS as required.
