Job summary

Homeward Bound of Marin is seeking a Program Coordinator to join its adult shelter and housing team in Novato, CA. You will provide housing-focused case management and coordinate services with healthcare, behavioral health, and community partners to improve housing stability for up to 24 adults.

The role requires strong documentation, trauma-informed practice, and collaboration with internal teams to ensure timely referrals and progress toward housing goals.

Qualifications

• Bachelor’s degree in social work, psychology, social welfare, or related field OR three years’ experience in a related residential or housing program.

Responsibilities

• Provide housing-focused case management in one-on-one settings for up to 24 single adults.
• Facilitate client groups and events across the housing portfolio.
• Incorporate intensive care coordination and housing stabilization into individualized plans with health and community partners.
• Identify and address barriers to housing stability (health, income, substance use, etc.).
• Respond to urgent and crisis situations with emergency services as needed.
• Provide supportive services, guidance, and referrals to community resources.
• Engage participants regularly to support housing stability and progress toward goals.
