Job summary

Join Homeward Bound of Marin as a Program Coordinator to support adults navigating housing stability. You will provide housing-focused case management, coordinate services with healthcare and community partners, and drive individualized care plans that promote safe, stable housing.

You will engage participants, document outcomes, and ensure timely data entry in HMIS/Salesforce, while collaborating with a dedicated team to end homelessness in Marin County.

Qualifications

• Bachelor’s degree in social work, psychology, social welfare, or related field, or three years of related residential/housing experience.
• Three or more years’ experience with under-resourced or low-income populations; homelessness experience preferred.
• Experience with Housing First and harm-reduction principles.
• Knowledge of SSI/SSDI/SDI; local resources.
• Experience with data entry systems (HMIS, Salesforce) and MS Office; proficiency with Adobe.

Responsibilities

• Provide housing-focused case management in one-on-one settings for adults across the housing team’s portfolio.
• Facilitate client groups and events across the housing portfolio.
• Develop, implement, and monitor individualized housing and care plans.
• Identify and address barriers to housing stabilization.
• Respond to urgent and crisis situations with relevant partners.
• Provide supportive services, guidance, and referrals to community resources.
• Engage participants regularly to support program participation and progress toward housing stability.
• Document housing and service outcomes to support program performance.
