Job summary

Quest Center For Integrative Health in Portland, OR seeks a Housing Case Manager (CRM) to support participants in our Outpatient SUD program. You will help clients secure safe, affordable housing, complete intakes, and coordinate with local agencies and peers to ensure continuity of care.

Successful candidates are committed, flexible, organized, and culturally competent, with the ability to build relationships, maintain records in EMR, and travel locally as needed to serve housing goals.

Qualifications

• Meets the definition of a Peer as defined in Oregon rules; PSS/CRM certification or willingness to become certified immediately.

Responsibilities

• Assist participants in meeting housing goals
• Complete intakes including move ins/outs
• Collect and submit random UAs
• Engage clients with person-centered approaches to build trust
• Assess housing needs and develop plan with goals
• Collaborate with local agencies for referrals and services
• Travel with clients to housing opportunities
• Maintain accurate records and submit documentation timely
• Work with an integrated team and adhere to laws and policies
