Job summary

Central City Concern in Portland, OR, is seeking a Case Manager I to join a multidisciplinary team providing case management and treatment readiness. You will help clients address crisis, health, housing, benefits, and employment opportunities.

Schedule: Mon–Fri, 8:00am–4:30pm. Location: Golden West, 707 NW Everett St. Compensation: $23.30–$30.91/hr; final based on experience. This is a union-represented role (AFSCME) with recall rights.

Qualifications

• High school diploma or GED and six months of experience in homelessness or related human services.
• Must pass DMV background check and be an acceptable driver per Fleet Safety Policy.
• Must pass pre-employment drug screen and background check.
• Ability to lift and perform physical tasks; must carry agency cell phone.

Responsibilities

• Engage clients through outreach to provide services non-coercively.
• Collect history through interview, records, and consent.
• Develop strengths-based service plans with clients.
• Coordinate with housing providers to plan residential and non-residential services.
• Participate in admission and discharge planning for hospitalization.
• Maintain client records per program standards and funding requirements.
