Responsibilities

• Conduct needs assessments related to housing, employment, medical care, insurance, and legal obligations.
• Coordinate with probation officers, DHS, housing providers, and community resources.
• Assist clients with insurance navigation and benefit activation.
• Support discharge planning and transition of care.
• Maintain accurate documentation in the electronic health record.
• Participate in staffing and treatment team collaboration.
• Advocate for clients while maintaining appropriate professional boundaries.

Key Responsibilities

• Conduct needs assessments related to housing, employment, medical care, insurance, and legal obligations
• Coordinate with probation officers, DHS, housing providers, and community resources
• Assist clients with insurance navigation and benefit activation
