Job summary

Catholic Charities SF HOME is seeking an experienced Case Manager to support homeless and low-income families, managing a caseload of 18-22 and helping them obtain and maintain permanent housing. Responsibilities include housing search assistance, crisis intervention, referrals, home visits, and collaboration with community services.

Requires BA or AA and experience with housing crises; bilingual Spanish/Cantonese preferred.

Qualifications

• BA degree and one year of experience preferred or AA degree and three years related experience.
• Previous experience locating and maintaining affordable family housing.
• Experience working with families in crisis.

Responsibilities

• Caseload portfolio assignment 18-22 clients; provide case management services.
• Housing search assistance and crisis intervention; referral to community resources.
• Develop and monitor individualized case plans with goals and timelines.
• Conduct home visits and coordinate with landlords to ensure housing stability.
• Maintain client files and data for monthly reports; input data into CARES/One System/Launch Pad.
