Job summary

Mission Services of Silicon Valley is seeking a Stabilization Case Manager II in San Jose. You will provide intermediate case management and participant support across housing, employment, and supportive services, using trauma‑informed practices and robust documentation.

You will coordinate with Career Navigators, Housing Navigators, and external partners, conducting intakes and developing personalized stabilization plans to move participants from crisis to stability.

Qualifications

• Bachelor's degree or equivalent experience required.
• 2+ years in workforce development, career coaching, or related services.
• Proficiency with MS Office, Google Workspace, and CRM/case management platforms.

Responsibilities

• Conduct comprehensive intakes, eligibility determinations, and barrier assessments.
• Develop stabilization, housing, or employment plans with measurable goals.
• Provide trauma-informed coaching, motivational interviewing, and crisis support.
• Maintain weekly or program-based participant meetings and document progress.
• Coordinate with internal teams and external partners to resolve barriers.
