Job summary

Mission Services of Silicon Valley is seeking a Stabilization Case Manager II to provide intermediate-level case management and participant support. You will manage more complex cases, coordinate housing, employment, and supportive services, and work with internal and external partners to promote long-term participant stability.

The role requires trauma-informed practices, high-quality documentation, and the ability to operate with greater autonomy across multiple program areas

Qualifications

• Bachelor's degree or equivalent in a related field.
• Minimum 2 years in workforce development, career coaching, or related services.
• Experience with CRM/case management systems and basic data entry.

Responsibilities

• Conduct intakes, determine eligibility, and assess barriers within program timelines.
• Develop stabilization/housing/employment plans with measurable goals.
• Provide trauma-informed coaching, motivational interviewing, and crisis intervention.
• Coordinate with internal teams and external partners to support participant success.
• Maintain 98% timely data entry and ensure audit-ready documentation.
