Job summary

City Net seeks Case Managers for on-site housing-focused case management at the Watt Ave Safe Stay Community Shelter. You will meet clients, assist with housing plans, and connect them to medical, mental health care, and permanent housing resources.

This in-person role requires empathy, strong communication, and precise data collection while working with diverse partners and schedules.

Qualifications

• Eligible to drive a company vehicle; insurance approval required.
• Empathetic, self-motivated, flexible and adaptable.
• Experience building relationships with stakeholders.
• Excellent professional boundaries and communication skills.
• Understanding of data collection and reporting.

Responsibilities

• Meet clients to prioritize housing and supportive services and build long-term housing plans.
• Help clients access housing programs and complete housing applications.
• Screen clients for eligibility and provide referrals to services.
• Advocate for clients with partner agencies to access resources.
• Collect and document data for HMIS/CES and funders.
• Meet productivity and outcome goals and comply with policies.
