Responsibilities

• Coordinate client care based on household strengths, identified needs, and barriers towards housing sustainability.
• Assist with eligibility documentation and funding processes.
• Develop collaborative SMART housing plans with staff and partners.
• Meet with clients weekly to monitor housing plan goals and conduct home visits.
• Document data in HMIS and maintain accurate records.
• Represent HealthNet professionally to internal and external partners.
