Job summary

HealthNet, Inc. is seeking a Case Manager I to coordinate client housing support, assess needs, and connect clients with community resources. You will develop SMART Housing Plans, document progress, and collaborate with internal teams and external partners to sustain housing stability.

The role requires strong communication, crisis awareness, and ability to work independently within a team-driven environment. Monday–Friday schedule with field and office work as needed.

Qualifications

• 3–5 years experience with vulnerable populations.

Responsibilities

• Meet weekly with clients to monitor Housing Plan goals.
• Create wrap-around Housing Plans with partners.
• Assist with eligibility documentation and housing funding.
• Conduct bio-psycho-social assessments to identify needs.
• Document all interactions and data in HMIS.
