Job summary

Cornerstones is seeking a Case Management Coordinator-Homeless Prevention and Rapid Rehousing to lead and supervise housing case management at the Embry Rucker Community Shelter. The role uses a Housing First approach to quickly exit individuals from shelters with tight collaboration across county and state partners.

The position requires a Bachelor's in a human services field (MSW preferred), supervisory experience, and strong trauma-informed communication skills.

Qualifications

• Bachelor’s degree in an applicable human services field or commensurate experience with homeless and/or at-risk populations.
• MSW plus two years post-Master’s degree experience a plus.
• Bi-lingual in English/Spanish, English/Arabic, English/French preferred.
• Ability to use lived experience of housing instability to inform assessments.

Responsibilities

• Supervise Homeless Prevention and Rapid Rehousing Case Managers.
• Manage a caseload of 12 households in Rapid Rehousing services.
• Coordinate assessments of housing barriers and needs for clients.
• Oversee HMIS data, outcomes, and reporting to meet contractual obligations.
• Provide clinical supervision and staff development, mentoring, and coaching.
• Engage with partner agencies to deliver holistic, integrated care.
• Schedule and manage workload to meet deadlines; flexible hours including evenings/weekends.
