Cornerstones seeks a Case Management Coordinator to oversee homeless prevention and rapid rehousing services at the Embry Rucker Community Shelter. You will supervise case managers, manage a 12-household caseload, and ensure data quality and reporting across HMIS.

Formación

• Minimum BS in human services or commensurate homeless service experience.
• 3+ years in homeless services with supervisory experience.
• Bi-lingual (English/Spanish, English/Arabic, or English/French preferred).

Responsabilidades

• Provide direct supervision for Homeless Prevention and Rapid Rehousing Case Managers.
• Manage a caseload of 12 households and coordinate housing barriers assessment.
• Oversee HMIS data quality, outcomes, and reporting.
• Coordinate with local agencies to ensure holistic care and rapid rehousing.
• Develop programs and evaluate services to meet contractual outcomes.
