Job summary

You will maintain confidentiality, coordinate with partner agencies, and assist clients in accessing resources and demonstrations of progress toward their goals. Strong organizational skills are essential.

Qualifications

• Experience working with the Seriously and Persistently Mentally Ill (SPMI) population, and/or equivalent experience.

Responsibilities

• Maintain professional relationships with clients and client confidentiality.
• Review documentation establishing tenants’ eligibility for programs and maintain files.
• Conduct initial intake and assessment of tenants and families’ needs.
• Prepare independent living plans with tenant input and set short/long-term goals.
• Locate community resources and make referrals to needed services.
